The EMH Mark-1, developed at Starfleet's Jupiter Station by Dr. Lewis Zimmerman, can treat virtually any injury or known disease, but can only function in areas equipped with holographic projectors.

The EMH program causes holographic projectors not only to generate an image of a person, but also to generate a magnetic containment field, within which electormagnetic energy is trapped, thereby giving the EMH the ability to physically manipulate objects such as patients and medical instraments.

The EMH is programmed with over five million possible treatments, with contingency options and adaptive programs utilizing sophisticated multitronic pathway programming. He is programmed with information from 2,000 medical references and the experience of 47 physicians. He also contains more than fifty million gigaquads of computer data and is equiped with the medical knowledge of more than three thousand cultures. His knowledge of medical treatments includes those based on psychospiritual beliefs such as those employed by some of Earth's Native Americans.

The EMH's program is extremely sophisticated, permitting him to learn from new data and experiences, and even be creative. The EMH is also capable of independant thought, and through his duties will follow the Hypocratic Oath to do no harm. In essence, this holoprogram is a sentient lifeform. He has limted control over his own activation and deactivation sequence, at his request. 

The Terra Nova's EMH has chosen the name "Dr. Twain", in honor of 19th Century Earth author, Samuel Clemons, whose pen name was Mark Twain. He is not programmed to bleed or feel any pain, nor is he programmed to cry. He is not equipped with genitalia.

The EMH is designed as a short-term suppliment for medical personnel, not to be used for more than 5,000 hours. When activated, the EMH establishes communication links with all key areas of the ship.

The Terra Nova's present EMH program originally served onboard the USS Enterprise-E, where it was installed just prior to the vessel's launch in 2372.

In 2373, when the Enterprise-E was invaded by the Borg, sickbay came under assimilation. However, despite Doctor Crusher's distaste for EMH's, the eventual Terra Nova EMH was afforded the opportunity to be a hero and distract the Borg long enough to allow the Enterprise crew to escape through a Jefferies Tube. He stalled them by offering them an analgesic cream, which they decided they didn't want and instead destabilized his program. He was shut down before being assimilated. The Doctor and her staff escaped.

Prior to the Enterprise's return to the present day, this EMH Mark-1 was removed from the ship, and put to work scrubbing waste disposal barges on Kelara Prime.

After the EMH Mark-1, which was assigned to the USS Voyager, returned to Earth in early 2377, many of the older EMH's were given certain program upgrades, including ones to give them longer service lives and returned to service. The Voyager's EMH successfully lobbied on their behalf, and shortly thereafter, the former Enterprise EMH was returned to service aboard the USS Terra Nova.

His first service date on the Terra Nova was on November 21, 2377 for pre-Commissioning inspection of the small Defiant Class vessel.... He serves onboard to this day, as the backup medical program, as well as the sickbay shift supervisor during the evening shift.
